To delete files or folders after cleaning with CCleaner on your Mac, you need to add them to the Include list in the Options section of the app. Here are the steps to do that https://support.ccleaner.com/s/article/including-files-and-folders-for-cleaning-with-ccleaner-for-mac?language=en_US.
In the left pane of CCleaner for Mac, click Options, and then click Include.
Add a new include rule by clicking Add.
To choose a volume or folder to include, select Volume or Folder, click Browse and then click the volume or drive. To choose a file to include, select File, click Browse, and then click the file.
If you choose a volume or folder, specify the file types to include, such as *.tmp or *.log.
Please choose the option that suits your needs: Include files only, Include files and subfolders, or Include files, subfolders, and the folder itself.
Click OK to save the rule.
After adding the files or folders you want to delete, ensure the Custom Files and Folders check box is selected on the OS X tab of the Cleaner pane. Then, click Analyze to scan for the files or folders, and click Run Cleaner to delete them.
