I've just downloaded the newest version of Defraggler (2.2.0.253) and tried to defrag a hard drive. After few hours of defragging, Defraggler just crashed. I started to defrag again and the same thing happened. I remember having this issue with some of the previous versions, too.
I'm using Windows Vista SP3 SP2.
In the Event Viewer I found this information:
exception code: 0xc000000d,
error offset: 0x000ed685
(same for both crashes)
I hope this to be solved soon.

How old is your Vista install?
Are you sure it is healthy/malware free?
Just wanted to see before they proceed to the next step.
Embedded rootkits can cause exceptions from time to time if they are present on a system, when certain functions are taking place.
You may have a healthy system, just asking, however, to be on the safe side first.

How did you get Vista SP3? It's not officially released, so I'm wary of what you have.
I have had Defraggler crashing similarly, scheduling a filesystem verification on boot is what fixed it.

XP has service pack 3 - that confused me...
I'm not aware of any malware on my computer.
My OS was installed in about 2008 but I install the updates from Windows Update regularly.
I'll try the disk check on the next boot.
OK, so after a boot-time disk check Defraggler still crashing... This doesn't seem to be the solution.
