If I select Internet Cache in Ccleaner menu, extensions information will delete too and extensions will work incorrect. Can You create selectable button for this.

Which version of Opera? Although if recent I'm not sure knowing that would make much difference.
I can't understand why deleting the "Internet Cache" locations would affect your extensions. Is it all extensions or a particular one?
Opera's extensions on my XP machine are stored in the same "LocalAppData\Opera\Opera" folder as the various cache subfolders, but in a separate "Widgets" subfolder.
Running an "Analyze" by right clicking the "Opera\Internet Cache" entry in CCleaner brings up the highlighted locations shown here ...
The "Widgets" folder isn't touched at all.
Can you confirm that your Opera extensions are in the correct location? And I believe the location is the same for XP, Vista and Win7. Not sure about Win8.
This problem appeared in 12 version of Opera. Windows Vista Home Edition x32.

If you're not sure how to check that, I'll explain it for you, and it isn't a problem as we all knew nothing about this stuff at one time.
Can you also try right clicking the "Internet Cache" entry in CCleaner and select "Analyze". A screenshot of the display window as shown in my first post above would be really helpful, as it will show the locations CCleaner is accessing.
Again, if you're not sure how to provide a screenshot I'll show you, or you could right click the display window and save the contents to a text file which by default is called "log.txt" and is saved to CCleaners "Program Files" folder.
You can upload that file to your post or copy and paste the contents.
Finally, are you using a "winapp2.ini" file with CCleaner, which is obtained from this forum and includes many extra user generated locations?
Or are you using any addons such as "CCEnhancer", or other cleaning programs?

I use only this program for cleaning.
I would say it seems pretty obvious looking at the CCleaner text file, that something being cleaned in the "widgets\wuid ..." folders is screwing up your extensions, although I can't say what exactly at the moment.
I can't do any testing with that immediately, as once I have my extensions installed and settings set, I then browse "Sandboxed", which means I'm not accumulating the same files in the "wuid" folders as you are. I don't have "activity.opr", "dcache.url", or "vlink4.dat".
For now, I would suggest setting the "Widgets" folder as an "Exclude" in CCleaner ...

... just simply to confirm that with "Widgets" folder excluded, your extensions work as they should after running CCleaner.
We can then try to isolate the files which are carrying settings besides the "psindex.dat" and "prefs.dat" files which aren't being cleaned.
If any of my Opera-using compadres can throw any light on the 3 above mentioned files in the "wuid" folders, I'm sure they will.
Let me know how the CCleaner "Exclude" turns out, and we'll take it from there.
