If this has started immediately after updating to the latest version, can you try the previous version which worked for you to make sure it still works.
If so, it will confirm that your problem is a version one.

How was it previously processing the Auto Start? As a Startup program or as a scheduled task? While both these methods still require UAC the latter, I believe, can be set to run automagically without prompting by using your password and username in the Task scheduler

Running CCleaner when Windows starts is something I've never done, so I've been confirming as to where CCleaner or Windows stores this setting, and it's in the following registry location ..
"HKEY_CURRENT_USER\Software\Microsoft\Windows\CurrentVersion\Run"
Can you check to see if you have that entry, and then check again after unchecking the "Run when the computer starts" box to confirm that it disappears, which it should do.
And then check the box again and look again to see if the registry entry appears again.
Just in case you don't know the "how to" ... go to "Start\Run" enter "regedit" (no quotes), press OK and navigate to the registry address shown above.
If you leave regedit open during these tests, you should go to "View\Refresh" in regedits menu each time.

So I find the line in regedit. And it removes when I uncheck, and add it when I check.
Just a difference with you screenshot : it call the 64bits version of CCleaner (I'm on a 64bits system) : "C:\Program Files\CCleaner\CCleaner64.exe" /AUTO

It sounds like the startup configuration is being set OK by CCleaner and Windows. You should be able to see the CCleaner startup entry in CCleaner itself ie "CCleaner\Tools\Startup", and also in "msconfig\startup".
That being the case, can you confirm that the ccleaner64.exe is actually in the correct location ..."C:\Program Files\CCleaner", and that it is the 64bit version in there. Also does CCleaner still run OK if you run it normally from a shortcut?
If all that's in order, then all I can think of is to carry out a clean reinstall of CCleaner.
If you have settings you want to keep, you can go to "Options\Advanced" and check the "Save all settings to INI file" box, and then copy the INI file from the CCleaner folder to a safe place. After the fresh install you can copy that file back to the CCleaner folder to restore your settings.
Lets see if that fixes the issue.

I can understand this in two different ways :-
1. CCleaner is not launched ;
2. CCleaner launches but does nothing.
I think that the /AUTO argument can cause confusion.
I believe it should result in CCleaner making no appearance on the desktop, other than the notification tray,
and Windows does not always display in the tray the things it should show - especially Windows XP.
Perhaps CCleaner is being launched but Windows is not showing it.
I suggest simply removing the /AUTO argument to observe what really happens.
